Rules for the protection of personal data and the sending of commercial messages

  1. The protection of personal data of a patient who is a natural person is provided by Act No. 110/2019 Coll., on the processing of personal data, as amended, and Regulation (EU) 2016/679 (GDPR Regulation).

  2. The dental office informs the patient that in order to fulfill the purchase contract and the contract for the provision of health care and the obligations arising from tax regulations and regulations relating to the provision of health care, it will only process the patient's personal data for as long as necessary, in particular: first and last name, surname number, residential address, e-mail address, telephone number, data on the health insurance company of which the patient is insured, data on the performance of medical procedures and the patient's medical documentation (hereafter all together as "personal data").

  3. If the patient has agreed to the processing of personal data for marketing purposes for an indefinite period, the dental office will store personal data, namely first and last name, residential address, e-mail address, telephone number for the purpose of sending marketing messages.

  4. The dental practice informs the patient that he can ask the dental practice (or the Personal Data Administrator) to correct and supplement personal data, limit the processing of personal data and delete personal data. Furthermore, the patient also has the right to object to the processing, as well as the right to the portability of personal data. In the event that the patient has agreed to the processing of personal data based on voluntary consent for the purpose of sending marketing messages, the patient has the right to withdraw this consent at any time, without prejudice to the legality of the processing of the personal data based on the consent granted before its withdrawal.

  5. The patient can entrust the processing of the patient's personal data to a third party as a processor. The dental office informs that it transfers personal data without the patient's consent, mainly for the purpose of fulfilling the contract to the health insurance company of which the patient is an insured and to state authorities based on a legal obligation or decision of a state authority, and further on the basis of a processing contract to the accounting office and laboratory technicians.

  6. The patient is entitled to ask how and when his personal data was handled.

  7. The dental office informs the patient that the supervisory authority, as far as the handling of personal data is concerned, is the Office for the Protection of Personal Data, www.uoou.cz . If the patient is convinced that his rights have been violated in connection with the violation of legal regulations governing the handling of personal data, he can file a complaint with the supervisory authority.
